APY Art Centre Collective Review
In July 2023 the State Government announced an Independent Review into the APY Arts Centre Collective (APYACC), to be conducted by Megan Krakouer, Anne Sibree and Cameron Costello.
This review has now concluded.
The Terms of Reference directed that the Panel review current and historical allegations and operations of the APYACC, and determine whether any matters should be the subject of investigation or regulatory action by another body.
The Panel has referred matters of governance and management practice to the Office of the Registrar of Indigenous Corporations (ORIC) and consumer protection and contracting matters to the Australian Competition and Consumer Commission (ACCC).
Review of Child Deaths at Munno Para and Craigmore
In 2022, two young children, aged seven and six, tragically died in circumstances that raised concerns about their health and wellbeing at the time of their deaths.
Separate to any criminal investigation, Mr Malcolm Hyde AO APM led a review into the multiple government agencies involved with the families of the two children who died.
The review considered the multi-agency arrangements in place to enable agencies to fulfil their responsibilities in protecting children from possible serious harm.
Review of child deaths at Munno Para and Craigmore

In February and July 2022, two young children, aged seven and six respectively, have tragically died in circumstances that have raised concerns about their health and wellbeing at the time of their deaths. Criminal investigations into the children’s deaths are underway. Both children and their families were known to multiple government agencies who were actively involved with them in recent years.
Separate to any criminal investigation, Mr Malcolm Hyde AO APM will lead a review into the multi-agency involvement with the families of the two children who died on 10 February and 15 July 2022.
The review will be focused on the arrangements related to the death of these two children, but will also examine any other matters that may be relevant to those arrangements.
The review will be directed at the effectiveness of multi-agency arrangements to enable agencies to fulfil their responsibilities in protecting children from possible serious harm.
The review will concentrate on the suitability and effectiveness of the systems applied by agencies for this purpose.
In particular, the review will:
- Identify the chronology and timelines of services delivered by agencies
- Outline the roles, responsibilities and interactions of the respective agencies
- Assess the appropriateness of systems in place, and the extent to which they are operating effectively
- Examine any other relevant matters
- Provide recommendations for any system improvements to better identify and protect vulnerable children.
The review is not intended to specifically find fault with individual persons in managing and delivering services in the two cases.
The review will be conducted in a manner which avoids interfering with criminal investigations.
The review will also have due regard to a number of other relevant reviews into government services for children and young people that are underway. These include:
- Independent post-coronial review being undertaken by Kate Alexander
- Foster and kinship care inquiry
- Commissioner’s inquiry into the Aboriginal child placement principle
- Review of the Children and Young People (Safety) Act 2017
These Terms of Reference may be reviewed to determine whether additional matters need to be explored subject to the SA Police investigations.
Agencies involved in service delivery to the families will be expected to provide the review with any required information and expertise. The Department of the Premier and Cabinet will support Mr Hyde in the conduct of the review.
Any outcomes of the review will be urgently considered by the government.
